Trauma-Focused Cognitive Behavioral Therapy

Trauma-focused cognitive behavioral therapy (TF-CBT) is one of the treatments offered at South Coast Behavioral Health. TF-CBT is an evidence-based therapy designed to help children and young adults with post-traumatic stress disorder (PTSD) or other trauma-related issues.

What Is TF-CBT?

Trauma-focused cognitive behavioral therapy (TF-CBT) is one of the treatments offered at South Coast Behavioral Health. TF-CBT is an evidence-based therapy designed to help children and young adults with post-traumatic stress disorder (PTSD) or other trauma-related issues.

The practice is a modified version of cognitive behavioral therapy (CBT). CBT is a psychotherapy that allows someone to explore the way they think, feel, and act to gain perspective and make improvements.

At South Coast Behavioral Health, we integrate TF-CBT into your unique treatment plan to help you recover from substance abuse and co-occurring mental health disorders. Our skilled therapists are trained in various therapy practices, including TF-CBT, to ensure your recovery is a safe and effective process.

We are open 24 hours per day, 7 days per week, 365 days per year.

What Does TF-CBT Treat?

While some children can display resiliency, there is evidence to suggest that those who experience trauma at a young age are at a higher risk for PTSD, depression, anxiety, substance abuse, and suicide attempts as adults. TF-CBT addresses the needs of individuals who have undergone trauma. That includes those who have experienced complex traumas such as physical abuse, sexual abuse, neglect, domestic violence, or traumatic grief.

TF-CBT is valuable for children and young adults. Data points to two-thirds of children experiencing several instances of trauma before they reach adulthood.  This can lead to negative emotional and behavioral responses to future situations as they age. The goal of TF-CBT is to reduce those negative responses using psychotherapy to develop life skills and coping mechanisms that can reshape negative thoughts and behaviors in the future.

TF-CBT helps with many different mental health disorders including:

  • Post-Traumatic Stress Disorder (PTSD): TF-CBT is particularly effective in reducing the symptoms of PTSD, such as flashbacks, nightmares, hypervigilance, and avoidance behaviors.
  • Anxiety Disorders: It can help children and adolescents manage anxiety disorders, including generalized anxiety disorder (GAD), specific phobias, and social anxiety disorder.
  • Depression: TF-CBT can address symptoms of depression, including sadness, hopelessness, and loss of interest or pleasure, which often come with traumatic experiences.
  • Substance Abuse: Trauma can be a risk factor for substance abuse and addiction. TF-CBT can address trauma-related triggers for substance use and help individuals in recovery.

TF-CBT also helps with other issues children or young adults may be having. Some of those issues include:

  • Behavioral Problems: Children who have experienced trauma may exhibit behavioral problems such as aggression, defiance, or conduct disorders. TF-CBT can help change these behaviors by addressing their underlying trauma-related causes.
  • Emotional Dysregulation: Trauma can lead to difficulty regulating emotions. TF-CBT equips people with skills to better manage their emotions and reduce emotional reactivity.
  • Self-Harming Behaviors: Some resort to self-harming behaviors as a way to cope with emotional pain resulting from trauma. TF-CBT can help them find healthier coping strategies.
  • Grief and Loss: TF-CBT can be adapted to address the complex grief and loss.
  • Attachment Issues: TF-CBT can help individuals rebuild secure and healthy attachment relationships.
  • Parent-Child Relationship Problems: TF-CBT can work on improving communication and understanding within the family.

What To Expect in TF-CBT Therapy?

TF-CBT incorporates cognitive-behavioral techniques, which means it addresses thought patterns and behaviors related to trauma. This includes helping individuals identify and challenge negative thoughts and beliefs related to their traumatic experiences. TF-CBT incorporates cognitive-behavioral techniques, which means it addresses thought patterns and behaviors related to trauma. This includes helping individuals identify and challenge negative thoughts and beliefs related to their traumatic experiences.

It often consists of a specific number of sessions, often ranging from 12 to 25. This makes it a more focused and goal-oriented therapy. Your therapist begins by building a trusting and supportive relationship with the client. Establishing rapport is crucial when working with children, as it helps create a safe and comfortable therapeutic environment. In a controlled and structured manner, the client is encouraged to share their traumatic experiences. This can be done through verbal storytelling, drawing, writing, or play. The therapist helps the client process and make sense of these memories.

For children and adolescents, the therapist often involves parents or caregivers in some sessions. They are educated about trauma, taught how to support their child’s recovery, and may participate in family-based interventions if needed. At the end of each session, the therapist often reviews the session’s content, summarizes key takeaways, and gathers feedback from the client regarding their experience and progress. You may be given homework assignments to practice the skills learned in therapy between sessions.

What Are the Benefits of TF-CBT?

TF-CBT is effective in helping children who have experienced trauma. It addresses the unique developmental needs of children and adolescents while providing a structured and evidence-based approach to healing from traumatic experiences.

Here’s how TF-CBT helps children:

  • Psychoeducation: TF-CBT begins by educating the child and their caregivers about the effects of trauma and how it can impact thoughts, emotions, and behaviors. This helps normalize their reactions and reduce feelings of shame or guilt.
  • Trauma Narrative: Children share their traumatic experiences during the session. This happens either through talking drawing, writing, or play. This helps them process the events and gain a sense of control over their memories.
  • Cognitive Restructuring: TF-CBT helps children identify and challenge negative thoughts and beliefs related to the trauma. By learning to replace irrational beliefs with more adaptive ones, children can reduce anxiety and distress.
  • Relaxation Techniques: Children learn relaxation and mindfulness techniques to manage anxiety, hyperarousal, and emotional distress. These skills provide them with tools to cope with intense emotions.
  • Affective Expression and Regulation: TF-CBT teaches children healthy ways to express and regulate their emotions. Through play, art, or other creative activities, children can explore and communicate their feelings.
  • Gradual Exposure: Controlled exposure to trauma-related memories and triggers helps desensitize children to distressing emotions and sensations associated with the trauma. This is often done in a gradual and supportive manner.
  • Safety and Coping Skills: Children learn practical coping skills to manage stress and anxiety. They develop strategies to handle triggers, manage difficult emotions, and solve problems.
  • Parent Involvement: TF-CBT involves parents or caregivers in the therapy process. They learn about trauma’s impact on their child and learn how to support their recovery at home. Parent-child communication and bonding are also promoted.
  • Enhancing Resilience: TF-CBT aims to build the child’s resilience and self-esteem. Children learn to view themselves as survivors rather than victims, empowering them to overcome challenges.
  • Family Communication: If the trauma has affected family dynamics, TF-CBT can address family conflicts and improve communication, fostering a more supportive and nurturing environment.
  • Gradual Reintegration: As the child progresses in therapy, TF-CBT focuses on helping them reintegrate into school, social activities, and other aspects of their life.
  • Endings and Closure: TF-CBT concludes with a focus on summarizing the progress made and discussing strategies for maintaining the gains achieved during therapy.
